When I activate the plug-in my admin panel went 404 page. Dont upload this plug-in.

Does what it says on the tin, no hassle involved.
'Pay by phone' appears on the checkout page and works perfectly. Orders are listed as 'Pay by phone' too so they can be followed up with a call.
