There are two ways to install Survey maker: the easy way, when you install Survey maker from your WordPress dashboard, and the not so easy way, when you install it from WordPress.org.

After one ofthis steps you can activate the plugin from the Plugins menu.

I was looking for a survey plugin that allows the redirection to a thank you page already with the free version, and it does this. As emailing the results to the user seems interesting and there are many more nice features, I will consider getting the full version.

Wow! That sums it up! This survey plugin does everything I needed. It replaces a well-known (and greedy) survey service for which I was paying a fortune, year after year. Now, I can do unlimited surveys with unlimited answers with all the special types of questions I needed. It works! Thanks so much! And then... I had one issue. They fixed it and sent me an updated version within 24 hours. Great service! And thanks for not making it yet another greedy plugin that requires monthly/annual subscriptions. Enough with those! Everybody wants to be a Netflix! I only use plugins I can 'buy' like this one. Many thanks!
